West Africa Battles Diphtheria Outbreaks
Authorities in several West African countries are trying to manage their huge diphtheria outbreaks, including in Nigeria, where millions are being vaccinated to cover wide gaps in immunity against the disease.
Authorities in several West African countries are grappling with managing large-scale diphtheria outbreaks. In Nigeria, the situation is particularly dire, with millions of people being vaccinated to address significant gaps in immunity against the disease. Since the current outbreak began in December 2022, at least 573 people have died out of the 11,640 diagnosed cases. However, officials believe the death toll could be significantly higher, considering the challenges in detecting cases across various states.

Widespread Outbreak
The highly contagious bacterial infection has spread to 20 out of Nigeria's 36 states. Niger and Guinea are also battling outbreaks, reporting 37 deaths out of 865 cases and 58 deaths out of 497 cases, respectively. Ifedayo Adetifa, head of the Nigeria Centre for Disease Control, described the current outbreak as the largest in recent history.
Low Vaccination Rates
A significant factor contributing to the high infection rate in the region is the historically low vaccination coverage. In Nigeria, only 42% of children under 15 years old are fully protected against diphtheria, while Guinea's immunization rate stands at 47%. Both figures fall well below the 80-85% rate recommended by the World Health Organization (WHO) to ensure community protection.
Vaccine Shortages
The situation is further exacerbated by global shortages of the diphtheria vaccine, as demand has surged in response to the outbreaks. Dr. Dagemlidet Tesfaye Worku, emergency medical program manager for Doctors Without Borders (MSF) in Abidjan, Ivory Coast, emphasized the urgent need for a massive scale-up of vaccination.
Government Efforts
The Nigerian government is taking steps to address the outbreak by ramping up vaccination efforts for targeted populations and assisting states in improving their capacity to detect and manage cases. However, several states continue to face challenges, including Kano, which accounts for over 75% of cases in Nigeria but has only two diphtheria treatment centers.
Challenges in Accessing Treatment
The limited number of treatment centers and the distances people must travel to access care pose additional challenges. This hinders effective management of the outbreak and contributes to the potential underestimation of the true impact of the disease.
The diphtheria outbreaks in West Africa highlight the importance of maintaining high vaccination rates to protect communities from infectious diseases. Governments and healthcare organizations must collaborate to address the vaccine shortages and strengthen immunization programs to prevent future outbreaks and mitigate the impact on affected populations.
